A Preliminary Report of 111 Cases of Laparoscopic CholecystectomuY



Abstract
Laparoscopic cholecystectomy(LC) has emerged rapidly as an acceptable and exciting new approach to cholelithiasis and it is performed under the visual control of television monitor without compromising the operative field. The benefits of LC is
not
only
minimizing the traumatic insult but also reducing the hospital stay and early return to normal activity. From April to December 1992, one hundread eleven cases of CL were performed at the Department of Surgery, Inje University Pusan Paik
Hospital.
The
most common indication was cholelithiasis with biliary colic(89), and other indications were acute cholelithiasis with cholelithiasis(17), acalculous chronic cholecystitis(1), tuberculous cholecystitis(1), and symptomatic polyp(3). Seven cases
were
converted to open cholecystectomy(two enforced conversions and five elective conversions). Operative complications were bile duct injury in one, bile leak in two, postoperative bleeding in five pateints and some other minor complications. Average
operation time was 90 minutes(range: 35 to 265 minutes). Average postoperative hospital stay was 3.5 days. In seventeen patients of acute cholecystitis we successufully completed LC in twelve patients(including eempyemas in four patients and
hydrops in
one), but complicated five cases were converted to open cholecystectomy due to blurred antomy. There was no operative death.
